Who We Partner With
We know that a referral is never just paperwork — it's a person, a family, and a plan that depends on reliable follow-through. Stellar Support Services works closely with county board SSAs, case managers, healthcare providers, school transition coordinators, and guardians to make sure every referral is handled with professionalism and care.
Our team is responsive, transparent, and easy to work with. When you refer an individual to us, you can trust that we'll communicate clearly, honor the support plan, and keep you updated on progress — because coordination works best when everyone is on the same page.
One point of contact. Clear communication. Reliable follow-through.
Who Can Refer
Service and Support Administrators play a critical role in connecting individuals to the right services. We're experienced with county board processes and know how to move referrals forward efficiently.
Whether you're coordinating a transition, working through a discharge plan, or connecting a client to long-term supports, we respond quickly and keep you informed throughout the process.
Physicians, therapists, and clinicians can count on us to honor care recommendations, coordinate supports appropriately, and communicate with respect for the individual's goals and medical needs.
Helping a student transition out of school-based services? We partner with schools and families to build a smooth bridge into adult services — with planning that starts before the transition date.
How It Works
Complete our online referral form, call (234) 788-3314, or email our intake team. Share what you know — even if your information is incomplete, we can help fill in the gaps.
We acknowledge every referral within one business day. If the situation is urgent, tell us when you submit it and we'll prioritize accordingly. You'll always have a named point of contact.
We review DODD eligibility, confirm county board coordination, and schedule an intake conversation with the individual and their authorized representative. If something's missing, we'll help coordinate it.
Once the person-centered plan is in place, services begin. You'll receive updates, progress notes, and clear communication — so the individual, family, and referral partner all stay informed.
